Lebanon Visa Application Form and Requirements of the Embassy of Lebanon

• Passport (signed and valid at least for 6 months with 2 blank visa pages); two colored photocopies of the passport ID page
• Lebanon Visa Application Form (filled out and signed)
• One recent Colored Passport size photo (2x2 inches)
• Proof of Legal Status in Canada for Non-Canadian applicants (PR Card, Work Permit, etc.)
• For Tourist/Visitor visa - work or school attestation (letter from the company you work for, school or university stating your identity, confirming your position witnessed and sighed)
• For Business visa - an introductory letter from Canadian employer indicating the nature of applicant's business, probable duration of stay, the places and firms to be visited, witnessed and sighed.
• The validity of a visa is 90 days starting from the issue date. The duration of a visa is one, three or six months starting from the day of entry to Lebanon
• A visa will not be issued on any passport or travel document stamped with an Israeli visa or seal
• Client Order form. Payments can be made by cash, money order or certified cheque. Prices do not include 13% HST
