Pakistan Visa Application Form and Requirements of the Consulate of Pakistan 

• Passport/Canadian Travel Documents (valid at least for 6 months with 2 blank visa pages)
• Pakistani Visa Application Form (filled out and signed)
• Two Passport-sized Colored photos  taken within past three months and stamped by a photographer
• Proof of Legal Status in Canada for Non-Canadian applicants (PR Card, Work Permit, etc.)
• For Business Visa – an introductory letter from the applicant’s company and an invitation letter from host/associate company/organization in Pakistan (copy of registration papers in Pakistan might be required) indicating the nature of applicant’s business, probable duration of stay, the places and firms to be visited
• Bank Statement
• Copy of flight itinerary
• Client Order form. Payments can be made by cash, money order or certified cheque. Prices do not include 13% HST.
