Ukrainian Visa Application Form and Requirements of the Ukrainian Consulate 

• Passport (signed and valid at least for 6 months after the date of the planned departure from the Russia with 3 blank visa pages).
• Ukrainian Visa Application Form properly filled out, printed and signed.
• Two Colored full front view passport sized photographs on a white background, taken within six months of the date of application and attached in the designed space of the visa application form.
  
• Proof of Legal Status in Canada for Non-Canadian applicants (PR Card, Work Permit, etc.)
• For Tourist Visa - Invitation/Visa Support documents. Tourist Invitation/Visa Support can be provided for an additional $60. This Visa allowed staying in Ukraine for less than 90 days within a 180 day period starting starting from the day of first entry to Ukraine.
  
• For Transit Visa - Documents confirming the transit nature of travel (a visa to the third country if it is required, a ticket, etc.) This visa allows transit through the territory of Ukraine within the period of 5 days.
  
• For Ling Term Visa - Invitation from a dually registered Organization or Work Permit. This visa can be issued for applicants who enters Ukraine with the purpose of obtaining documents permitting staying in Ukraine for the period of more than 90 days. Within 45 days from the moment of entry to Ukraine a holder of long-term one entry visa should apply to relevant authorities in Ukraine for a permanent/temporary resident permit.
